Whether you or a loved one is in need of care, our primary goal at Evergreen is to help you or your family member maintain their independence and quality of life.
Our services are flexible, specific to your individual needs and preferences. We are available evenings, weekends and holidays. Evergreen offers a variety of short-term and long-term services, a few hours a week up to 24-hours a day, seven days a week. Your customized plan of care, personalized just for you, may include but not be limited to the following services:

• Respite Care
  We are available to stay with homebound clients and offer minimal assistance for short periods of time while the regular caregiver is away.
  
• Homemaking Services
  Our staff can assist with light housekeeping such as vacuuming, dusting, cleaning floors, taking out the trash, laundry, and meal preparation. We also provide grocery shopping & errands, transportation to appointments, and bill paying services, if needed.
  
• Personal Care
  We offer assistance to anyone who needs help with daily routines such as, personal hygiene, bathing, dressing, oral hygiene, toileting assistance, incontinence care, mobility assistance, exercise and assistance with eating.
  
• Medication Management
  Under the supervision of our Registered Nurse, we provide medication review, set-up, administration and monitoring. We coordinate medication management closely with physicians, pharmacists and family to ensure client compliance.
  
• 24-Hour Companion Service
  Evergreen offers round-the-clock, one-on-one companionship and supportive services to help anyone remain in their home. 24-hour companionship can include homemaking, personal care and medication management services.
  
• Specialized Care
  Our staff are experienced and qualified in provision of specialized care for individuals on Hospice services or persons diagnosed with dementia or Alzheimer’s. Our staff are knowledgeable and understand specific illnesses and diseases such as hypertension, heart disease, ALS, MS, diabetes, and cancer and how they impact our client’s in-home care.
  
• Registered Nursing Services
  Evergreen’s Registered Nurse provides assessments, evaluations, medication management & coordination, and supervision of staff.
  
• Care Management, Consultation, Referral and Placement Services
  We help individuals assess their needs, align them with community resources, then coordinate and provide a range of services to meet those needs. We can refer you to senior care professionals and help you identify and explore housing options. Our years of experience and education have given us the expertise and knowledge to assist you with navigating the senior care system.
  

How do you get started?

• Contact us. We are always happy to talk with you to answer any questions about our services. Give us a call or email us and we can begin the conversation.
  
• The Home Visit. We will conduct a home visit to evaluate, assess and assist you in determining what services you may need and design a plan that fits within your budget.
  
• Your Service Plan. A personalized service plan is jointly developed with you to direct our staff.
  
• Disclosure Agreement. Clients and/or responsible parties sign a service agreement and disclosure after receiving and understanding Clients Rights & Responsibilities, policies, services and pricing.
  
• Caregiver Assignment. Caregivers are carefully selected who possess the skills, experience and personality best suited to your needs. Clients are encouraged to interview any caregiver we refer to you. It is our priority to make sure our caregivers are a perfect fit for you!
